Required Documents

The following documents are required before moving forward with your next steps:


  • Certified Driving Record from the State of New Hampshire DMV

  • Certified Driving Record from other states (if applicable)

  • Arrest Report

  • Court Order

Certified Driving Record

  • Change the text and add your own content, including any information that is relevant to share.

    If you had a DUI in New Hampshire, or you had a New Hampshire drivers license when you had a DUI in another state, you will need to order your certified New Hampshire driving record. If you have been licensed in New Hampshire, you order it online, and within 3-5 business days they mail it to you (or to us if you like) and then you upload to the portal. If you have not been licensed in New Hampshire, you will need to print the form below and mail it in with $15 check or money order.

     If you have ever had a DUI in another state, you will also need to obtain that state’s driving record. New Hampshire also requires that you turn in the driving record for any state you were licensed in when you had the DUI. Below are the most common states people need driving records from.

    For clients with a DUI in Maine:
    Please either visit a Maine BMV and obtain a 10-year certified driving record in person or contact the Maine BMV at 207-624-9000 x 52116 to request a certified driving record to be sent by mail. The record must be certified to be accepted by our office.

Arrest Report

You will need to contact the police department that arrested you for the DUI.



Request from the police department your "discovery" - that is a packet that includes your Arrest Report and Narrative as well as the Form of Refusal or the BAC or Blood tests, your criminal record, and compliant/charges at arrest.  Upload the PDF, or pictures of each page to the portal.

Court Order

You will need to supply either the Court Order of Conviction  - or if you don't have that you can order a Case Summary from the courts.


Following conviction of DUI/DWI/OUI - you are given paperwork that day which is the Court Order of Conviction. If your conviction is in another state, you will need to call the court where you were convicted if you do not have the court order.



If you have lost your NH Court Order, you can order it for free - and it will be emailed to you the same day you call the NH Courts. Call 1-855-212-1234 and request your DWI Case Summary. When they email you the PDF, please upload to the portal.

Criminal Record

You will need a Criminal Record from the State of NH.



Criminal Record Request - cost is $25 debit/credit online

When you order criminal record online, it is emailed to you or to us within 1-2 business days.  When you get into the website - click the "Request Online NH Criminal Conviction Check" box - The first box that you enter info is REQUESTER information:  This is your information and it will be emailed to you with a link to access the download. If you are having a hard time downloading the file, you can give us a call and forward the email to us so we can try to download.

Proof of Hardship

You will need proof if you receive TANF, SSDI, SSI, APTD or VA Disability benefits and you are a NH resident.



Once you provide proof of being on one of the following, then you will qualify for hardship discounts. Proof must be submitted prior to intake/screening along with proof of residency.  

Victim Impact Panel (VIP) Requirement

All IDCMP clients convicted on or after January 1, 2026 are required to complete a court-approved Victim Impact Panel (VIP) prior to scheduling their 20-hour class.



The Victim Impact Panel must be completed online and paid for by the client. These panels focus on the real-life impact of alcohol and drug-related driving on individuals, families, and communities.
