1. About this policy. This policy explains how Chrysalis Recovery Center, LLC ("Chrysalis," "we," "us") handles personal information collected through our website and our text messaging. It does not replace our Notice of Privacy Practices, which explains how we protect and use your health information.

2. Information we collect. We collect the information you give us, such as your name, phone number, email address, and the details you enter in forms or tell us when you contact us. When you use our website contact form, you tell us who you are and why you are contacting us, and what you write is sent to our intake team through a form-delivery service.

Our website also collects some information automatically. We use Google Analytics, which uses cookies to record which pages are viewed, how long visits last, the type of device used, where a visit came from, and approximate location. It does not receive what you type into our forms. Our home page shows a public visitor counter provided by a third-party service. The counter does not use cookies, but the service receives the visitor's IP address. If you play the video on our home page, YouTube may set its own cookies. Our home page shows our Google reviews, which are loaded from Google. When you follow a link to our intake or client portal pages, you leave our website and IntakeQ's own policies apply. You can control cookies through your browser settings. Our website host keeps standard server records, which include IP addresses.

3. How we use it. We use your information to provide and schedule our services, to respond to you, to send the scheduling, case management and document-reminder messages described below, to meet legal and billing requirements, and to keep our systems secure.

4. Text messaging (SMS). We send scheduling, case management and document-reminder text messages to existing clients who agreed to receive them in our intake paperwork. Our staff and counselors may also text with clients directly. Our SMS Terms and Conditions are at chrysalisrecovery.com/sms-terms. We do not share your mobile number or your text-message consent with third parties or affiliates for marketing or promotional purposes. You can opt out of text messages at any time by replying STOP.

5. Who we share information with. We do not sell your personal information. We share it only with the service providers that help us run our practice, such as our scheduling and records system and our phone and text-message provider, under agreements that require them to protect it, and when the law requires or allows us to. Health information is also governed by our Notice of Privacy Practices.

6. Security. We use reasonable safeguards to protect your information. No method of sending information over the internet or by text is completely secure. To send us documents or health information, please use our secure client portal or secure email, or call us at 603-998-4210. If you choose to text us a document, see section 10 of our SMS Terms and Conditions.

7. How long we keep it. We keep client records for 10 years. We keep other information only as long as needed to provide services and meet legal and record-keeping requirements.

8. Your choices. You can stop text messages by replying STOP. You can contact us to ask about your information, to correct it, or to ask how it is used, at the contact details below.

9. Children. Our website is not directed to children under 13, and we do not knowingly collect information from them through it.

10. Changes. We may update this policy. The date at the top shows when it last changed.
